A PCO medical is a mandatory part of the PCO licence application. Driving for long hours requires good physical health, alertness, and the ability to respond safely in different situations. As part of the PCO application, TfL requires PHV drivers to meet the DVLA Group 2 medical standards.

The TPH204 is the official medical form used when you apply for PCO licence approval or renew an existing PCO driver licence. The medical must be completed and signed by a GMC-registered GP. This confirms that you are medically fit to work as a licensed PHV driver.

How Does the PCO Medical Exam Work

You can complete the TPH204 form as part of a full PCO licence application package or as a PCO medical-only service. To start, you will provide your medical history. The GP then carries out the required checks.

We provide access to GMC-registered GPs and manage the process to ensure your PCO application goes smoothly. Same-day appointments are subject to availability. The medical appointment is on average 30-40 minutes.

PCO Medical Test assessments

Once the checks are complete, the GP signs and stamps the TPH204 medical form. If we are handling your PCO licence application, the form is submitted as part of the process. If you are applying on your own, the completed form is given to you to submit.

PCO Licence: DVLA Group 2 Standards for Drivers with Disabilities

Drivers with disabilities are allowed to apply for PCO licence approval, provided the DVLA Group 2 medical standards are met. TfL assesses each PCO driver licence application individually. In many cases, vehicle adaptations allow drivers to meet the required safety standards.

Hand-operated braking controls
Hand-operated accelerator control
Reconfigured foot pedal controls
Infra-red control panel for operating vehicle lights, indicators, and horn

TPH204 PCO Medical Test for PCO Licence Renewals

Previous Standards

Previously, private hire and taxi drivers were assessed under DVLA Group 1 medical standards. Group 2 standards applied mainly to larger passenger and commercial vehicles.

Updated Requirements

TfL now applies DVLA Group 2 medical standards to all PHV drivers. This applies whether you are submitting a new PCO licence application or renewing an existing PCO driver licence.

Renewal Process

Drivers aged between 45 and 65 must renew their Group 2 medical clearance every five years. A completed PCO medical is required each time you renew your licence.
